Resident Care Navigator
We are seeking a Resident Care Navigator to ease the transition into long-term care, providing emotional, logistical, and healthcare support. In this role, you will also support the Director of Facility and Resident Care by assisting with administrative duties, coordinating resident care programs, and ensuring compliance with facility policies. If you have a background in social work or Licensed Practical Nurse (LPN), this role is an opportunity to make a meaningful impact!
Key Responsibilities
- Resident Support & Advocacy:
- Act as a liaison between residents, families, and the healthcare team, ensuring smooth transitions and high-quality care.
- Care Coordination:
- Conduct resident assessments, care plan development, and care conferences.
- Administrative & Financial Support:
- Assist with resident admissions, financial reconciliations, funding applications, and record maintenance.
- Leadership & Compliance:
- Support nursing staff, ensure compliance with long-term care licensing requirements, and participate in committees (Resident & Family Councils, Risk Management, Pharmacy Review, etc.).
- Quality Assurance & Crisis Management:
- Provide emotional support, counseling, and crisis intervention as needed.
Qualifications & Experience
- Bachelors in Social Work (preferred) or LPN with care coordination experience.
- Strong problem solving, organizational, and communication skills. Ability to collaborate within a multidisciplinary team and manage high-stress situations.
- Current First Aid & CPR certification required.
- Proficiency in Microsoft Word and Excel.
- Familiarity with long-term care licensing requirements, financial processes, and resident-centered care practices.
Work Conditions
Regular interaction with residents, families, and healthcare professionals.
Facilitate resident admissions on weekends.
Fast-paced work setting requiring critical thinking and quick decision-making.
May require working extended hours or covering shifts in urgent situations.
