The Resident Care Coordinator will be an integral part of the health care team that provide residents with medical oversight with direction from a Physician.  The Care Coordinator actively participates in the nursing process with great autonomy in administrative, consultative, and case management.  This position will assess, plan, implement, and evaluate the specific needs of each resident, utilizing innovative and current gerontological practices.  The Care Coordinator will establish and maintain positive working relationships with management, staff, residents, and residents’ families.
