Direct Care Manager (DCM)
We are seeking a Direct Care Manager (DCM) for a non-unionized leadership position supporting the Director of Facility and Resident Care in overseeing clinical operations, staff management, and resident care services. As a Registered Nurse (RN), the DCM ensures compliance with regulatory standards, promotes high-quality care, and fosters a culture of excellence in resident-centered services. This leadership role provides mentorship to staff, enhances clinical processes, and ensures efficient workflow in a collaborative, interdisciplinary environment.
Key Responsibilities
- Leadership & Staff Management
- Provide mentorship, training, and support to nursing staff.
- Assist with scheduling, recruitment, and performance evaluations.
- Participate in on-call rotation to provide after-hours clinical and operational support.
- Resident Care & Clinical Oversight
- Oversee and develop individualized resident care plans.
- Conduct assessments (InterRAI, MMSE) and participate in physician rounds.
- Support infection prevention and control (IPAC) initiatives.
- Work as an RN to cover the floor when necessary.
- Compliance & Quality Assurance
- Ensure adherence to licensing requirements and accreditation standards.
- Conduct audits and quality improvement reviews.
- Monitor resident health status and respond to concerns promptly.
- Education & Professional Development
- Provide training and support for staff on best practices in resident care.
- Promote continuous learning and professional growth among the care team.
- Interdisciplinary Collaboration & Committee Participation
- Serve on committees such as Infection Control, Pharmacy Review, and Quality Improvement.
- Foster communication between residents, families, and healthcare professionals.
- Administrative & Management Support
- Act as a resource for staff, residents, and families.
- Support leadership in policy implementation and workforce planning.
Qualifications & Experience
- Registered Nurse (RN), registered with NSCN (BSN preferred).
- 3-5 years of leadership experience in long-term care or geriatric settings.
- Strong mentorship and team collaboration skills.
- Knowledge of regulatory standards, quality assurance, and care planning.
- Proficiency in Microsoft Office and electronic health records (EHR).
- Current First Aid and CPR Certification.

Required to participate in on-call rotation for after-hours clinical and operational support.
Fast-paced work setting requiring critical thinking and quick decision-making.
May require working extended hours or covering shifts in urgent situations.
